This position works as a member of an ACT (Assertive Community Treatment) Team. This position is based in a clinic/agency location but services are primarily provided in community locations where individuals need services such as their homes, work sites, shelters and neighborhood health clinics. Employees are required to provide or arrange transportation to these locations via public transportation or personal vehicle (mileage and bus/train fare are reimbursed for required work-related local travel). This position makes up part of a 10-12 person multidisciplinary treatment team consisting of a psychiatrist, nurse, certified peer specialist, behavioral health care clinicians and paraprofessionals who provide comprehensive and intensive behavioral health care services and supports for up to 100 individuals.
The Professional Registered Staff Nurse is responsible for providing age-appropriate, culturally and ethnically sensitive care, maintaining a safe environment, educating patients and their families about healthy practices and treatment modalities, assuring continuity of care, coordinating the care across settings and among caregivers, managing information, communicating effectively, and utilizing technology. The Staff Nurse utilizes the nursing process which includes the components of assessment, nurse diagnosis, outcomes identification, planning and coordination, implementation, and evaluation. The role of the staff nurse also includes patient advocacy, teaching, performance improvement, leadership and professional development. Registered nurses with less than 1 year experience are required to participate in a curriculum based residency program which supports evidence based practice and is designed to complement hospital and nursing orientations and specialty training.
Previous work on an ACT Team strongly preferred. Candidates with previous experience providing behavioral health care services in the community strongly preferred.
